ABSTRACT

BACKGROUND: : The II Brazilian Consensus on Gastric Cancer of the Brazilian Gastric Cancer Association BGCA (Part 1) was recently published. On this occasion, countless specialists working in the treatment of this disease expressed their opinion in the face of the statements presented. AIM: : To present the BGCA Guidelines (Part 2) regarding indications for surgical treatment, operative techniques, extension of resection and multimodal treatment. METHODS: To formulate these guidelines, the authors carried out an extensive and current review regarding each declaration present in the II Consensus, using the Medline/PubMed, Cochrane Library and SciELO databases initially with the following descriptors: gastric cancer, gastrectomy, lymphadenectomy, multimodal treatment. In addition, each statement was classified according to the level of evidence and degree of recommendation. RESULTS: : Of the 43 statements present in this study, 11 (25,6%) were classified with level of evidence A, 20 (46,5%) B and 12 (27,9%) C. Regarding the degree of recommendation, 18 (41,9%) statements obtained grade of recommendation 1, 14 (32,6%) 2a, 10 (23,3%) 2b e one (2,3%) 3. CONCLUSION: : The guidelines complement of the guidelines presented here allows surgeons and oncologists who work to combat gastric cancer to offer the best possible treatment, according to the local conditions available.

RESUMO Racional: O II Consenso Brasileiro de Câncer Gástrico da Associação Brasileira de Câncer Gástrico ABCG (Parte 1) foi recentemente publicado. Nesta ocasião inúmeros especialistas que atuam no tratamento desta doença expressaram suas opiniões diante declarações apresentadas. Objetivo: Apresentar as Diretrizes da ABCG (Parte 2) quanto às indicações de tratamento cirúrgico, técnicas operatórias, extensão de ressecção e terapia combinada. Métodos: Para formulação destas diretrizes os autores realizaram extensa e atual revisão referente a cada declaração presente no II Consenso, utilizando as bases Medline/PubMed, Cochrane Library e SciELO, inicialmente com os seguintes descritores: câncer gástrico, gastrectomia, linfadenectomia, terapia combinada. Ainda, cada declaração foi classificada de acordo com o nível de evidência e grau de recomendação. Resultados: Das 43 declarações presentes neste estudo, 11 (25,6%) foram classificadas com nível de evidência A, 20 (46,5%) B e 12 (27,9%) C. Quanto ao grau de recomendação, 18 (41,9%) declarações obtiveram grau de recomendação 1, 14 (32,6%) 2a, 10 (23,3%) 2b e um (2,3%) 3. Conclusão: O complemento das diretrizes aqui presentes possibilita que cirurgiões e oncologistas que atuam no combate ao câncer gástrico possam oferecer o melhor tratamento possível, de acordo com as condições locais disponíveis.

ABSTRACT Background: Since the publication of the first Brazilian Consensus on Gastric Cancer (GC) in 2012 carried out by the Brazilian Gastric Cancer Association, new concepts on diagnosis, staging, treatment and follow-up have been incorporated. Aim: This new consensus is to promote an update to professionals working in the fight against GC and to provide guidelines for the management of patients with this condition. Methods: Fifty-nine experts answered 67 statements regarding the diagnosis, staging, treatment and prognosis of GC with five possible alternatives: 1) fully agree; 2) partially agree; 3) undecided; 4) disagree and 5) strongly disagree A consensus was adopted when at least 80% of the sum of the answers "fully agree" and "partially agree" was reached. This article presents only the responses of the participating experts. Comments on each statement, as well as a literature review, will be presented in future publications. Results: Of the 67 statements, there was consensus in 50 (74%). In 10 declarations, there was 100% agreement. Conclusion: The gastric cancer treatment has evolved considerably in recent years. This consensus gathers consolidated principles in the last decades, new knowledge acquired recently, as well as promising perspectives on the management of this disease.


RESUMO Racional: Desde a publicação do primeiro Consenso Brasileiro sobre Câncer Gástrico em 2012 realizado pela Associação Brasileira de Câncer Gástrico (ABCG), novos conceitos sobre o diagnóstico, estadiamento, tratamento e seguimento foram incorporados. Objetivo: Promover uma atualização aos profissionais que atuam no combate ao câncer gástrico (CG) e fornecer diretrizes quanto ao manejo dos pacientes portadores desta afecção. Métodos: Cinquenta e nove especialistas responderam 67 declarações sobre o diagnóstico, estadiamento, tratamento e prognóstico do CG com cinco alternativas possíveis: 1) concordo plenamente; 2) concordo parcialmente; 3) indeciso; 4) discordo e 5) discordo fortemente. Foi considerado consenso a concordância de pelo menos 80% da soma das respostas "concordo plenamente" e "concordo parcialmente". Este artigo apresenta apenas as respostas dos especialistas participantes. Os comentários sobre cada declaração, assim como uma revisão da literatura serão apresentados em publicações futuras. Resultados: Das 67 declarações, houve consenso em 50 (74%). Em 10 declarações, houve concordância de 100%. Conclusão: O tratamento do câncer gástrico evoluiu consideravelmente nos últimos anos. Este consenso reúne princípios consolidados nas últimas décadas, novos conhecimentos adquiridos recentemente, assim como perspectivas promissoras sobre o manejo desta doença.

INTRODUCTION: Gastric gastrointestinal tumors (GIST) are a rare and usually asymptomatic neoplasm that can present as abdominal mass in more advanced scenarios. Since surgical resection is the main aspect of the treatment, locally advanced tumors require multivisceral resection and, therefore, higher postoperative morbidity and mortality. OBJECTIVE: To perform a review the literature on the topic, with emphasis on the neoadjuvant therapy. METHODS: Literature review on the Medline database using the following descriptors: gastrointestinal stromal tumors, neoadjuvant therapy, imatinib mesylate and molecular targeted therapy. RESULTS: Surgical resection remains the cornerstone for the treatment of GISTs; however, tyrosine kinase inhibitors have improved survival as an adjuvant therapy. More recently, neoadjuvant therapy have been described in the treatment of locally advanced tumors in order to avoid multivisceral resection. CONCLUSION: Despite surgical resection remains as the most important aspect of the treatment of GISTs, adjuvant and neoadjuvant therapy with tyrosine kinase inhibitors have shown to both improve survival and resectability, respectively.

INTRODUCTION: Gastric emptying (GE) and food tolerance (FT) can be altered after Roux-en-Y gastric bypass (RYGB) has been performed, especially when it involved the use of a restrictive mechanism (such as a silastic ring). AIM: To assess GE and FT in patients who underwent banded (BRYGB) or non-banded Roux-en-Y gastric bypass (RYGB). METHODS: Forty-seven BRYGB patients and 47 RYGB patients underwent gastric emptying scintigraphy (GES) and FT assessment (by means of a questionnaire) between 6 months and 2 years postoperatively. RESULTS: GES was performed on average 11.7 ± 5.0 months (6 to 24) postoperatively. T½ medians (time taken for the gastric radioactivity to decrease to half of the original value in the gastric pouch) in the RYGB and BRYGB groups were 48.7 min (40.6-183.0 min) and 56.3 min (41.1-390.9 min), respectively (p = 0.031). The median of total questionnaire scores was 24 points (18-27) in the RYGB group and 20 points (13-27) in the BRYBG group (p < 0.001). CONCLUSIONS: The band (silastic ring) delays GE time and does not affect patient satisfaction or food tolerance to vegetables, bread, or rice, but does affect tolerance to the intake of meat, salad, and pasta. The best tolerated foods are vegetables, salad, and fish. Banded patients are more likely to regurgitate and vomit. Gastric emptying does not affect FT.

RESUMO Introdução: O tumor estromal gastrintestinal (GIST) gástrico é neoplasia que cursa, por vezes, com apresentação assintomática, se manifestando como massa abdominal, relacionada a casos mais avançados. Esses, por sua vez, exigem tratamento com operações maiores e que cursam com mais alta morbimortalidade. Objetivo: Revisar e atualizar o tratamento do GIST gástrico, com enfoque na relevância do tratamento neoadjuvante. Método: Revisão da literatura utilizando a base de dados Medline/PubMed. Utilizaram-se os seguintes descritores: gastrointestinal stromal tumors, neoadjuvant therapy, imatinib mesylate e molecular targeted therapy. Dos artigos selecionados, 20 foram incluídos. Resultados: O tratamento cirúrgico é pilar fundamental para a cura do GIST. Entretanto, após a introdução do mesilato de imatinibe, classicamente utilizado como terapia adjuvante, houve mudança no manejo do GIST, permitindo aumento da sobrevida e diminuição da recorrência. A aplicação como terapia neoadjuvante é mais recente, e visa evitar procedimentos maiores sem, no entanto, prejudicar o resultado oncológico. Conclusão: A ressecção cirúrgica possui papel bem estabelecido no tratamento do GIST, inclusive com abordagem laparoscópica. O tratamento adjuvante com mesilato de imatinib, durante os primeiros três anos após a operação, mostra-se como opção segura para casos com elevado risco de recidiva. A terapia neoadjuvante é opção promissora para casos de tumor localmente avançado, permitindo ressecções menores e com menor morbimortalidade operatória.

BACKGROUND: The purpose of this study was to assess the tolerance of early oral feeding after total laryngectomy. METHOD: A randomized multicenter study was conducted that included 89 individuals subjected to total laryngectomy. The participants were allocated to 2 groups: early (n = 44), early oral feeding; and late (n = 45), late oral feeding. The participants in the early group were assessed as to acceptance of oral feeding, and their food intake was quantified. RESULTS: In the early group, the total energy expenditure and protein needs were not met through oral feeding alone at any time during the first 7 postoperative days. The times to attain 25% and 50% of the total energy expenditure and protein needs by oral feeding after surgery were 4 and 7 days, respectively. CONCLUSION: The patients subjected to early oral feeding failed to meet their caloric and protein needs through that route alone during the first 4 postoperative days and required complementary nutrition through another route. © 2015 Wiley Periodicals, Inc. Introduction: Lymph node (LN) metastasis is a major staging criterion of gastric cancer (GC). GC prognosis is associated with the number of metastatic LNs. Objectives: To evaluate the impact of using a lymph node revealing solution (LRS) in gastrectomy specimens. Materials and methods: prospective study in 30 gastrectomy specimens (19 men, average age of 59.5; 11 women, average age of 66.1) for pathological tumor, node, metastasis (TNM) staging. After LN dissection according to the routine procedures (conventional method [CM]), the tissue was emerged in LRS: a mixture containing 65% ethanol, 20% ether, 5% acetic acid, and 10% formalin solution (10%), for 36 hours with 3 changes. A new LN dissection was performed. The number of LNs obtained from both methods was compared and analyzed. The number of metastatic and non-metastatic LNs was determined. Results: From 1,005 (33.5/specimen) dissected LNs, 657 of which (21.9/specimen) by the CM, and 348 (11.6/specimen) after using LRS (p = 0.0002), metastases were detected in 272 LNs (9.2/case), 211 of which (7.0/specimen) were found with the CM, and 61 (2.0/specimen) after using LRS (p = 0.0028). The number of LNs increased 53.0% with LRS, and the number of metastatic LNs was 28.9% higher. The pN classification changed in 5 (16.7%) of the 30 specimens, and in stage grouping in 4 (13.3%) analyzed cases. Conclusion: The use of LRS in gastrectomy specimens is simple, enables dissection, increases the number of LNs, and occasionally changes pN staging and stage grouping. .


Introdução: O envolvimento do linfonodo (LNs) por metástase é importante critério para estadiar câncer gástrico (CG). Relaciona-se o prognóstico do CG com número de LNs metastáticos. Objetivos: Avaliar o impacto do uso de solução reveladora de linfonodos (SRL) em espécimes de gastrectomia. Materiais e métodos: Estudo prospectivo em 30 espécimes de gastrectomia (19 homens, média de idade 59,5 anos, e 11 mulheres, média de idade 66,1 anos) para estadiamento patológico tumor-linfonodo-metástase (TNM). Após dissecção dos LNs de acordo com procedimento de rotina (método convencional [MC]), esse tecido foi imerso em SRL, mistura contendo 65% de álcool, 20% de éter, 5% de ácido acético e 10% de formal a 10%, por 36 horas, com três mudanças, sendo realizada nova dissecção de LNs. Os LNs obtidos por ambos os métodos foram comparados e analisados. O número de LNs metastáticos e sem metástases foram determinados Resultados: Dissecados 1.005 LNs (33,5/caso): 657 LNs (21,9/caso) pelo MC e 348 LNs (11,6/caso) após uso da SRL (p = 0,0002). Metástases foram detectadas em 272 LNs (9,2/caso): 211 (sete/ caso) pelo MC e 61 (dois/caso) após uso de SRL (p = 0,0028). O uso da SRL aumentou o número de LNs em 53%, e o número de LNs metastáticos, em 28,9%. Houve mudança na classificação pN em cinco (16,7%) dos 30 espécimes, e no estadiamento por grupos em quatro (13,3%) casos analisados Conclusão: O uso de SRL em espécimes de gastrectomia é procedimento simples de aplicar, facilitando a dissecção, aumentando o número de LNs e eventualmente mudando estadiamento pN e de grupamento. .

ABSTRACT

Introdução: O retalho miocutâneo infrahioideo (RMI) foi descrito por Wang em 1986. É utilizado em Cirurgia de Cabeça e Pescoço para reconstrução após ressecção de tumores, com sua principal indicação em tumores de andar inferior de boca. Objetivo: Avaliar, de forma retrospectiva, a exequibilidade e confiabilidade do RMI. Método: RMI foi utilizado na reconstrução cirúrgica em 25 pacientes portadores de carcinoma de andar inferior de boca. Apenas um dos cirurgiões do grupo tinha experiência prévia com a técnica. Os demais realizaram um único procedimento sob supervisão, a partir do qual os realizavam de forma independente. Resultados: Em três casos (12,0%) houve deiscência de sutura entre o retalho e o leito receptor, formação de fístula salivar e consequente deiscência de sutura no sítio doador. Em quatro pacientes (16,0%) houve epidermólise, com descamação e posterior reepitelização, sem necessidade de reintervenção cirúrgica.a Em dois pacientes (8,0%), houve necrose total da pele no terço distal do retalho, tratada com debridamento local. Não houve necrose muscular do RMI e não ocorreram complicações tardias. Comparando-se os três cirurgiões, não se observou diferenças nas taxas de complicações. Conclusão: O RMI é exequível no nosso meio, pois não demanda material específico e tem rápida curva de aprendizado. Os cirurgiões com vivência na especialidade podem facilmente incorporar essa técnica em sua rotina de reconstrução. As taxas de complicações são aceitáveis. O RMI é útil para reconstrução em cirurgia de cabeça e pescoço, notadamente para tumores iniciais de assoalho de boca e língua oral.


Introduction: The musculocutaneous infrahyoid flap (MIF) was described by Wang in 1986. It is used in reconstruction on head and neck surgery, with its main indication in tumors of the mouth floor. Objective: Evaluate retrospectively, the feasibility and reliability of the MIF. Method: The MIF was used in the surgical reconstruction in 25 patients with squamous cell carcinoma of the buccal floor. Only one surgeon of the group had prior experience with the technique. The other two surgeons performed the first procedure under supervision, then they performed independently. Results: Three cases (12.0%) had dehiscence between the flap and the recipient site, salivary fistula formation and subsequent wound dehiscence at the donor site. Four patients (16.0%) had epidermolysis with flaking and subsequent re-epithelialization without need for surgical intervention. In two patients (8.0%), there was complete necrosis of the distal skin flap treated with debridement site. There was no muscle necrosis of MIF and there were no late complications. Comparing the three surgeons , no differences in complication rates was observed. Conclusion: MIF is feasible in our country because it does not require special equipment and has fast learning curve . Surgeons with experience in the art can easily incorporate this technique into your routine reconstruction. Complication rates are acceptable. The MIF is useful for reconstruction of head and neck surgery, especially for tumors initial floor of the mouth and oral tongue.

OBJETIVO: avaliar a eficácia da complementação por coagulação com plasma de argônio para reduzir a taxa de neoplasia residual ou recorrente após ressecção endoscópica completa fragmentada de grandes adenomas sésseis colorretais. MÉTODOS: critérios de inclusão: pacientes com grandes adenomas colorretais sésseis, > 20mm, sem sinais morfológicos de infiltração profunda, submetidos à ressecção endoscópica completa fragmentada estudada com cromoendoscopia e magnificação de imagens. Os pacientes foram randomizados em dois grupos: grupo 1 - nenhum procedimento adicional e, grupo 2 - complementação por coagulação com plasma de argônio. O seguimento por colonoscopia foi realizado em três, seis e 12 meses de pós-operatório. Foi avaliada a taxa de neoplasia residual ou recidiva local. RESULTADOS: foram incluídos no estudo um total de 21 lesões. Onze lesões no grupo 1 e dez no grupo 2. Ocorreram duas neoplasias residuais ou recorrências locais em cada grupo, detectadas em três meses de acompanhamento. CONCLUSÃO: a complementação por coagulação com plasma de argônio após uma aparente ressecção endoscópica completa em fragmentos de grandes adenomas sésseis colorretais não parece reduzir a ocorrência de lesão adenomatosa residual ou recidiva local.

OBJECTIVE: To evaluate the incidence of pharyngocutaneous fistula after total laryngectomy and try to identify its predictors. METHODS: From May 2005 to April 2010, 93 patients underwent total laryngectomy. We evaluated complications during and after surgery and compared them with the following variables: gender, nutritional status, previous tracheotomy, tumor location, type of surgery, TNM staging, prior treatment with chemotherapy and/or radiotherapy, use of flaps for reconstruction and surgical margin. All patients presented with advanced neoplastic disease according to TNM. RESULTS: 14 (15.1%) patients developed postoperative salivary fistula. The mean time to onset of salivary fistula was 3.5 days, with a standard deviation of 13.7 days. Comparing salivary fistula with TNM variables, type of operation and neck dissection, prior tracheotomy, use of flap, preoperative radio and chemotherapy and surgical margin, there was no statistically significant difference (p> 0,05). CONCLUSION: The incidence of salivary fistula was 15.1% and no predictive factor for its formation was found.

OBJETIVO: Avaliar a incidência de fístula faringocutânea após laringectomia total e tentar identificar os fatores preditores. MÉTODOS: No período de maio de 2005 a abril de 2010, 93 pacientes foram submetidos à laringectomia total. Foram avaliadas as complicações per e pós-operatórias e comparadas com as seguintes variáveis: sexo, estado nutricional, traqueostomia prévia, localização do tumor primário, tipo de operação realizada, estadiamento de acordo com o TNM, tratamento prévio com quimioterapia e/ou radioterapia, utilização de retalhos para reconstrução e margem cirúrgica. Todos os pacientes apresentavam a neoplasia em estádio avançado segundo o TNM. RESULTADOS: 14 (15,1%) pacientes evoluíram com fístula salivar no pós-operatório. O tempo médio de aparecimento da fístula salivar foi 3,5 dias, com desvio padrão de 13,7 dias. Comparando a fístula salivar com as variáveis TNM, tipo de operação e esvaziamento cervical, traqueostomia prévia, utilização de retalho miocutâneo, rádio e quimioterapia pré-operatória e margem cirúrgica, não foi observado diferença estatisticamente significativa (p>0,05). CONCLUSÃO: A incidência de fístula salivar foi 15,1% e não foi encontrado fator preditor para sua formação.

OBJETIVO: Avaliar a incidência e os fatores preditores da hipocalcemia e hipoparatireodismo definitivo pós-tireoidectomia. MÉTODOS: Foi dosado cálcio iônico no pré e no pós-operatório (primeiro, segundo e 30ºdia) em 333 pacientes submetidos à tireoidectomia. Naqueles que apresentaram hipocalcemia, as dosagens foram feitas também aos 90 e 180 dias de pós-operatório, quando se dosou também o paratormônio. Os pacientes foram agrupados segundo a presença ou ausência de hipocalcemia e avaliados segundo idade, sexo, função tireoidiana, volume tireoidiano, número de paratireoides identificadas e necessidade de reimplante de paratireoides, tipo de operação, tempo operatório e diagnóstico histopatológico. RESULTADOS: A incidência de hipocalcemia temporária foi de 40,8% (136 pacientes), e hipoparatireoidismo definitivo de 4,2% (14 pacientes). Tireoidectomia total ou reoperação, esvaziamento cervical, hipertireoidismo, tempo operatório e idade acima de 50 anos foram fatores determinantes de incidência significativamente maior de hipocalcemia e hipoparatireodismo definitivo (p<0,05). CONCLUSÃO: os fatores preditores da hipocalcemia pós-operatória incluem idade (>50 anos), tireoidectomia total, reoperação, esvaziamento cervical e tempo operatório. Os fatores preditores do hipoparatireoidismo definitivo pós-tireoidectomia incluíram tipo de operação, diagnóstico histológico e hipertireoidismo.

RACIONAL: O controle do sangramento na hepatectomia é um desafio para os cirurgiões. A clampagem do pedículo hepático é manobra cirúrgica que pode promover redução do sangramento, mas provoca isquemia hepatocelular. Isso, junto com a reperfusão depois que a clampagem termina, leva à lesão de isquemia e reperfusão. OBJETIVO: Examinar os efeitos da lesão de isquemia e reperfusão no fígado após clampagem contínua e intermitente do pedículo hepático, usando a quantificação de apoptose como ferramenta. MÉTODO: Vinte coelhos New Zealand foram divididos em grupos 1 (controle), 2 (60 minutos de isquemia contínua) e 3 (60 minutos de isquemia intermitente alternando 12 minutos de isquemia e três minutos de reperfusão). Biópsias hepáticas foram colhidas antes e ao fim da isquemia e após seis horas de reperfusão, quando os animais eram sacrificados. Os fragmentos obtidos foram submetidos à análise histológica e histoquímica (reação de Tunel). Campos microscópicos foram analisados para caracterização e quantificação de apoptose. RESULTADOS: A isquemia levou à elevação do índice apoptótico em ambos os grupos experimentais em relação aos controles, mas similar entre eles. Depois da reperfusão os índices voltaram aos valores iniciais. CONCLUSÃO: A clampagem do pedículo hepático, tanto contínua quanto intermitente, induz a apoptose em células hepáticas de modo igual.

RESULTS : Ischemia led to an increased apoptotic index in both experimental groups in comparison to controls, but similarly between them. After the reperfusion, the indexes returned to baseline values. CONCLUSION: Clamping of the hepatic pedicle, either continuous or intermittent, induces apoptosis in liver cells in a similar way.

Cisplatin (CDDP) is one of the most active cytotoxic agents and has been widely used in the treatment of peritoneal carcinomatosis by the intraperitoneal (i.p.) route. However, CDDP, a low-molecular-weight compound, is rapidly absorbed by the capillaries in the i.p. serosa and transferred to the bloodstream, inducing the appearance of systemic side-effects, such as nephrotoxicity. Furthermore, the i.p. CDDP chemotherapy is limited to patients whose residual tumor nodules are less than 0.5 cm in diameter after surgical debulking. The failure of i.p. therapy is attributed to the poor penetration of CDDP into larger tumors. One strategy to improve drug delivery in the peritoneal region and reduce toxicity is the use of drug delivery systems. The objective of the present work was to evaluate the biodistribution and antitumoral effect of long-circulating and pH-sensitive liposomes containing CDDP (SpHL-CDDP), as compared with free CDDP, after their i.p. administration in Ehrlich ascitic tumor-bearing mice. After administering a 6 mg/kg single i.p. bolus injection of either free CDDP or SpHL-CDDP, ascitic fluid (AF), blood and organs (kidneys, liver, spleen and lungs) were collected and analyzed for CDDP content. The area under the CDDP concentration-time curve (AUC) obtained for AF and blood after SpHL-CDDP administration was 3.3-fold larger and 1.3-fold lower, respectively, when compared with free CDDP treatment, thus indicating its high retention within the peritoneal cavity. The determination of the ratio between AUC in each tissue and that in blood (Kp) showed a lower accumulation of CDDP in kidneys after SpHL-CDDP treatment. The SpHL-CDDP treatment demonstrated a significant uptake by the liver and spleen. SpHL-CDDP treatment led to a higher survival rate of mice with initial or disseminated peritoneal carcinomatosis than CDDP treatment. These results indicate that SpHL-CDDP may be useful for i.p. chemotherapy due to their greater concentration in the peritoneal cavity.
